Video and Photo Waiver: I give Next Step Dance Studio my permission for the public display of pictures and or video of my child for possible advertisement purposes

Medical Emergencies: In case of emergency or sudden illness, I hereby give authority to any hospital or doctor to render immediate emergency aid as it might be required for the undersigned student's health and safety. Next Step Dance Studio DOES NOT carry medical insurance for its students. It is required that all students be covered by their own family insurance and if injury occurs, it is understood that the students own policy is your only source of reimbursement. I hereby declare the participant to be in good physical and mental health.

Release of Liability: I certify my child has no ailments of disabilities that would prevent my child from participating in Next Step Dance Studio activities. As the legal parent or guardian, I release and hold harmless Next Step Dance Studio, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while in or upon the premises or any premises under the control and supervision of Next Step Dance Studio, its owners and operators or in route to or from any of said premises.

The Party: Our parties are one hour and thirty minutes in length. Our professional dance instructors will also serve as your personal hostess for the entire party. The instructor will lead a fun dance warm up, offer dance games, and teach a fun dance that matches your theme. NO DANCE EXPERIENCE NECESSARY

Parents provide: Birthday cupcakes, candles and drinks

All party attendees will need to complete a waiver by a parent in order to participate.

No alcohol is allowed on studio premises

Studio provides table setting: general birthday plates, forks, napkins and tablecloth

Cost: $250 for 10 guests (birthday child is free), add $10 for each additional guest - Extra charges for additional requests will be accepted

A $50 deposit is required at the time of booking to hold the day/time of party. Balance due 1 week before party date.
